If you wish to register your child for preschool, please download and complete the following:

1. 2011-2012 Preschool Registration
2. Child Care Enrollment
3. Health History and Emergency Care Plan
4. Child Health Report - signed and dated by a physician within thirty days after your child starts preschool
5. Day Care Immunization Record - due within thirty days after your child starts preschool
6. Emergency Card - available at the Center and due on or before the first day of class
