If you wish to register your child for preschool, please download and complete the following:
- 2010-2011 Preschool Registration
- Child Care Enrollment
- Health History and Emergency Care Plan
- Child Health Report - signed and dated by a physician within thirty days after your child starts preschool
- Day Care Immunization Record - due within thirty days after your child starts preschool
- Emergency Card - available at the Center and due on or before the first day of class
